Patients with ischemic heart disease may present across different clinical coronary disease scenarios, like acute coronary syndromes (with and without ST segment elevation), chronic coronary syndrome and in special situations (such as diabetes mellitus, chronic renal insufficiency, atrial fibrillation among others), which may get benefit from different antithrombotic schemes. The main objective is to reduce ischemic events without increasing risks (essentially hemorrhagic), for which a careful risk-benefit assessment is required, leading to a variety of therapeutic strategies and corresponding publications, with the aim of preventing either ischemic and hemorrhagic events. Our objective is to show updated evidence-based treatment in the mentioned different clinical scenarios; along with recommendations for the management of one of the most threatening complications, bleeding.
