Type A retrograde aortic dissection is a rare entity, in which the best therapeutic option must be chosen when complications arise. A patient with type A retrograde dissection is presented, with typical appearance of back pain, who was started with medical treatment as the first therapeutic measure and after 48 hours after finding pleural pericardial effusion in tomographic control, the behavior was reconsidered, opting for combined surgical resolution and hemodynamics.
