HFpEF in Argentina: what the ArFey-Preser Registry teaches us

Abstract

The ArFey-Preser Registry, Argentina's first prospective, multicenter study of heart failure with preserved ejection fraction (HFpEF), enrolled 638 patients across 18 provinces between December 2022 and May 2024. The cohort—predominantly older women (mean age 72 years, 62.1%) with a substantial cardio-renal-metabolic comorbidity burden—showed 7.3% annual mortality, 11.5% heart-failure hospitalization, and 90.3% survival within 24 months. This editorial situates these findings against international registries and trials (PARAGON-HF, ESC-HF III, ESC-HF-LT, JASPER, OPTIMIZE-HF/GWTG-HF, RECOLFACA, EDIFICE-Mx, I-PREFER) and concludes that, although a gap remains in the use of SGLT2 inhibitors and mineralocorticoid receptor antagonists relative to current guidelines, Argentine prescribing is not below the international standard, and its pace of adoption compares favorably. ArFey-Preser's chief contribution is establishing a contemporary national baseline against which future therapeutic progress can be measured.
