Perfil clínico, manejo y evolución de la insuficiencia cardíaca con fracción de eyección preservada en Argentina: Resultados finales del Registro ArFey-Preser

Introducción: La insuficiencia cardíaca (IC) con fracción de eyección preservada (ICFEp) está subrepresentada en registros argentinos. Los objetivos de este reporte fueron analizar perfil clínico, manejo y evolución de ICFEp en Argentina. Material y métodos: ArFey-Preser fue un registro multicéntrico, observacional que incluyó pacientes ambulatorios o pre-alta hospitalaria con fracción de eyección del ventrículo izquierdo ≥50%, signos-síntomas de IC y evidencia de cardiopatía estructural. Los puntos finales fueron mortalidad y hospitalización por IC al año y supervivencia al final del estudio. Resultados: Entre diciembre/22 y mayo/24, se incluyeron 638 pacientes, edad 72,4±11,3 años, 62,1% mujeres, FEVI 60,5±6,4%. Las comorbilidades más prevalentes fueron: hipertensión arterial 90%, enfermedad renal 54,2%, obesidad 50,3%, diabetes 44,5%, fibrilación/aleteo auricular 40,4%, anemia 36,7% y coronariopatía 26%. La clase funcional fue 3-4 en 34,2% y hospitalizaciones previas por IC en 58,6%. La evaluación incluyó: ecocardiograma 100%, péptidos natriuréticos 57%, búsqueda de isquemia 39,3%, cinecoronariografía 58,8%, métodos adicionales 6,9%. La medicación basal fue inhibidores del cotransportador sodio glucosa 2 (iSGLT2) 42,3%, antagonista mineralocorticoideo (ARM) 42,3%, inhibidores del sistema renina-angiotensina (iSRA) 82,4% y betabloqueantes (BB) 79,6%; con incremento al año en iSGLT2 17,2% y reducción de BB -8,6% e iSRA -8,1 (todos p<0,001). la mortalidad anual fue 7,3% y hospitalización por IC 11,5%; con supervivencia a 24 meses 90,3%. Conclusiones: La ICFEp en Argentina se caracterizó por predominio de mujeres, añosos, multimorbilidad y eventos frecuentes. La búsqueda de coronariopatía fue elevada, con bajo uso de métodos adicionales. La subutilización de iSGLT2 y ARM indica una brecha con recomendaciones actuales.
