Anomalous origin of the coronary arteries (AOCA) is a rare congenital malformation, with clinical relevance primarily determined by the anatomical course of the affected vessel. While many patients remain asymptomatic and the anomaly is discovered incidentally, certain patterns—such as an interarterial course between the aorta and the pulmonary artery—have been associated with an increased risk of myocardial ischemia and sudden cardiac death, particularly in young athletes.
