Abstract

Introduction: Congenital malformations of the coronary arteries affect between 1 and 5.6% of the general population and include various conditions characterized by the abnormal origin or course of any of the 3 main epicardial coronary arteries.

Clinical Case: A 62-year-old female patient presented to the emergency department reporting severe precordial pain radiating to the left arm. Acute myocardial infarction was diagnosed, and coronary angiography revealed an anomalous origin of the right coronary artery from the left main coronary artery, with a total occlusive lesion in its middle segment.

Conclusions: Individualized treatment of patients with coronary anomalies, especially when associated with severe manifestations such as myocardial infarction, is the treatment of choice in these cases.
